Chronic low back pain (CLBP) remains an incredibly burdensome condition globally, with rising recognition of multiple axial pain generators contributing to symptoms. Two increasingly well-characterized sources are vertebrogenic pain and sacroiliac joint (SIJ) dysfunction. Vertebrogenic pain is linked to Modic type I/II changes and mediated via the basivertebral nerve. SIJ dysfunction is often mechanical, instability related, degenerative, or post-fusion biomechanics. This report describes simultaneous BVNA and SIJ fusion with a single compressive lateral transfixing implant.
