Introduction: A 70-year-old male with history of right C6 radiculopathy developed right upper extremity paralysis and left upper extremity weakness following C6/7 interlaminar epidural steroid injection (ILESI). Recommendations for cervical ILESI were followed in this procedure, however, initial and follow imaging showed both intramedullary gas in the spinal cord along with evidence of spinal cord infarction.
