Managing Hip Arthroplasty with Regional Anesthesia in Patient with Severe Sarcoidosis and Cardiac Dysfunction

Introduction: Patients with a LVEF < 30% and pulmonary hypertension (RVSP > 50 mmHg) face high perioperative risk. Neuraxial anesthesia, particularly continuous spinal anesthesia (CSA), allows for titrated dosing and improved hemodynamic control. We report a 54 yoF with sarcoidosis, ILD, pulmonary hypertension (RVSP 58 mmHg), and LVEF 25% undergoing total hip arthroplasty. CSA combined with preoperative PENG block enabled stable anesthesia with minimized hemodynamic shifts, thus offering a safe, effective strategy for high-risk surgical patients.
