Tarlov cysts are cerebrospinal fluid-filled perineural lesions, typically found incidentally in the sacral spine 1. While usually asymptomatic, larger cysts can cause back pain, leg weakness, or bowel/bladder dysfunction 1. Initial treatment is conservative, with options including CT-guided aspiration with or without fibrin glue injection, cyst fenestration, nerve root imbrication, and epidural steroids2,3. Here, we present the first reported case of a Tarlov cyst-induced sacral pain being exacerbated during pregnancy.
