Introduction: A 52-year-old female with DM1, ESRD on hemodialysis, PAD, and bilateral below-knee amputations presented to the pain clinic with severe bilateral phantom limb pain. This case highlights the effectiveness of perineuromal bupivacaine based peripheral nerve blocks in managing refractory phantom limb pain in medically complex patients, underscoring the potential role of high-volume LA with steroid injections compared to dextrose based hydrodissection for acute phantom limb pain.
