Intercostal peripheral nerve stimulation for chronic post-mastectomy and post-thoracotomy: A medically challenging case

Introduction: Intercostal neuralgia is a debilitating complication of thoracic surgery causing persistent rib pain and allodynia. Postmastectomy pain syndrome (PMPS), by contrast, results from injury to extrathoracic nerves and presents with breast or chest pain and dysesthesia. While Intercostal and pectoralis/serratus (PECS) blocks may provide temporary relief, durable treatment options are limited. We present a case of combined intercostal neuralgia and PMPS following right mastectomy and lobectomy, successfully treated with a 60-day intercostal peripheral nerve stimulator.
