Introduction: Spinal cord stimulator (SCS) infections are rare but serious, often requiring device explantation. In immunosuppressed patients, infection risk is heightened due to poor wound healing and bacterial clearance. While standard treatment includes removing the implantable pulse generator (IPG), capsulectomy remains underexplored in pain medicine. Other fields suggest it may help resolve device-related infections (1-3). This case series presents two immunosuppressed patients who underwent SCS explantation with capsulectomy, highlighting its potential role in high-risk populations.
