Effective perioperative pain control in trauma patients can be complicated by limited history and individualized based on patient specific factors. We present a case of a young male with flail chest and multisystem trauma whose severe pain and agitation revealed previously undiagnosed opioid use disorder. This case highlights the role of methadone in managing both acute pain and withdrawal while emphasizing the importance of a multidisciplinary approach to optimize outcomes in complex trauma care.
