Vertebral hemangiomas are commonly incidental findings and are rarely symptomatic, but may present technical challenges during interventional spine procedures when coupled with vertebral endplate changes.¹ Basivertebral nerve ablation has emerged as an effective treatment for chronic vertebrogenic back pain, particularly in patients with Modic changes.²,³ Hemangiomatous bone, characterized by softened tissue and unpredictable electrical impedance, poses unique procedural difficulties for ablation, requiring technique modification.⁴,⁵
