Some patients with failed back surgery syndrome (FBSS) do not respond to traditional spinal cord stimulation (SCS), including high-frequency and burst waveforms. For these nonresponders, subthreshold (paresthesia-free) SCS using low-amplitude, low-frequency settings may offer effective pain relief. Unlike conventional strategies focused on frequency, emerging evidence suggests that pain modulation in subperception SCS depends more on precise control of amplitude and pulse width, reframing the therapeutic approach as one of targeted “neural dosing.”
