This case highlights the need to consider a wide differential diagnosis for chronic periscapular pain. While rib fractures in athletes, particularly swimmers, are rare, reported incidences range from 0.58 to 1.66 percent athlete exposures. This highlights the importance of awareness and a broad differential diagnosis. Initially, myofascial syndrome and scapulothoracic bursitis were suspected given the clinical findings, but MRI findings clarified the diagnosis.
