Introduction: Distinguishing between cervical spine and shoulder pathologies as sources of pain and dysfunction presents a significant clinical challenge. Accurate differentiation between cervical conditions, such as radiculopathy, and shoulder pathology is essential, as certain conditions—such as cervical myelopathy—require urgent management. This case highlights a complex patient presentation in which cervical myelopathy developed insidiously in a patient who initially presented with shoulder pain.
