Opioid-induced pruritus (OIP) is a common side effect of mu-opioid receptor agonists and often resists antihistamines. Nalbuphine, a kappa agonist and mu antagonist, provides both pain relief and itch prevention. Two ED cases highlight its utility: low-dose nalbuphine with hydromorphone prevented pruritus in a sensitive patient, while a higher dose served as primary analgesia. Both achieved effective pain control without pruritus or sedation, supporting nalbuphine as a safe, evidence-informed alternative for acute care.
