Introduction: Autologous iliac crest bone grafting is commonly performed in orthopedic and spine surgery. A frequent complication is postoperative pain at the posterior iliac crest, often due to injury or entrapment of the superior cluneal nerves, an under-recognized condition known as cluneal neuralgia. These nerves are particularly susceptible during posterior graft harvesting. Despite its clinical relevance, cluneal nerve-mediated pain is often mistaken for nonspecific low back pain, leading to delays in diagnosis and appropriate treatment.
