Introduction: Patient is an 83-year-old male not on anticoagulation with a history of urothelial cancer status post resection, and chronic opioid therapy (hydrocodone 5mg QID) presenting with acute left shoulder pain. He was recently seen multiple times in the ED due to spontaneous left hip and left elbow hematomas in which hematologic workup was largely unremarkable. CT imaging of the left shoulder revelaednumerous rim-enhancing intramuscular collections along with axillary lymphadenopathy.
