Chronic femoral neck nonunion is a rare but debilitating complication of hip fracture, typically necessitating surgical correction due to ongoing pain and biomechanical instability. However, some patients decline operative treatment, creating a clinical dilemma requiring a multidisciplinary, patient-centered strategy. This case highlights successful conservative management of femoral neck nonunion in an elderly patient through a coordinated pain management and rehabilitation approach.
