Adjacent Segment Disease (ASD) refers to new radiculopathy or myelopathy occurring at spinal levels adjacent to a prior fusion site. While pain in the lumbar region or lower extremities may be reduced with presence of intrathecal drug deliver system (IDDS), on occasion, there may be natural progression and subsequent exacerbation of lumbar ASD. This case report illustrates a patient who required lumbar fusion extension for their ASD while avoiding removal of their IDDS.
