Introduction: Chronic low back pain (CLBP), a leading cause of disability, involves overlapping etiologies. Two under recognized contributors include vertebrogenic pain maintained via the basivertebral nerve and neuromuscular dysfunction from lumbar multifidus atrophy. FDA approved therapies basivertebral nerve ablation (BVNA) and multifidus stimulation demonstrate independent effectiveness. However, evidence on their combined use in mixed structural and functional pathology is limited. This report describes successful treatment with BVNA and multifidus stimulation in a patient with Intractable CLBP.
